Because they are built as lectures, not as learning. Courses that respect a clinician's time — short segments, one clear objective each, immediate clinical relevance — get finished. Instructional design is the difference, not willpower.
Three things: a specific behaviour to change, practice against realistic cases, and a reason to revisit the material. A certificate proves attendance; changed Monday-morning decisions prove learning.
